When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.

Register at our GP Practice

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available from reception if you need one.

We never discriminate on the grounds of age, sex, medical condition, race, disability or social class, and if we are unable to register you we will always provide you with a reason in writing.

If you are already registered with us and move out of the area you will be asked to move to a new practice closer to your new address.

Temporary Services

If you are visiting the area or here for only a short period of time, and need to see a doctor, please contact reception on 01253 229470. You will then be asked to complete the relevant form.

Complete a temporary registration form

Once you have completed the form you can receive emergency treatment from the practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the practice.

Your Named GP

All Patients registered with the surgery will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the surgery. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
