Sickness Certificates (Fit Notes)

You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home Visits

House visits are only available for patients who are housebound because of illness or disability.

If you need a home visit, you can help us by calling reception before 10am.

A doctor may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Surgery.

If you wish to simply speak to the doctor over the phone, please contact reception and explain the nature of the call. Reception staff will then arrange a suitable time for you to speak to the doctor.

The receptionist will then triage to the on-call GP who will then contact you or triage you to the appropriate clinician.

Video Group Clinics

What are Group Video Clinics?

On-line Group clinics are a tried and tested way for doctors to see up to 10 patients with similar conditions supported by either a GP, Social prescriber, pharmacist, practice nurse or other clinician.

What are the benefits of online group clinic?

  • more time with your GP
  • meet others with similar concerns and health conditions
  • time to talk about what really matters to you
  • receive prescriptions, arrange tests and referrals

People tell us that group clinics can help them to feel more confident to manage their health, allow more time to talk about what matters and find out more about their condition.

What Happens with video group clinic?

Before a group clinic you will receive information with frequently asked questions. You maybe asked to obtain some up-to-date bloods, blood pressure, mood scores, lifestyle scores, weight etc.

When you join online, our facilitator welcomes everyone into the group meeting room and will ask a couple of questions to check your identity. Our facilitator will then explain the process of the group consultation, runs through some group understandings and supports everyone to come up with their key concerns or questions for discussion around the health topic of concern. The clinician will answer questions that you have and will have discuss your individual concern.

Out of Hours

Enhanced Access - Evenings and Weekends

Important

An appointment must have been booked in advance of visiting the service.

Evening and weekend appointments are available with GPs, practice nurses and other healthcare professionals for routine pre-booked services.

The Fylde Coast NHS is committed to improving access to primary care services. This includes making appointments available at times that are convenient to patients.

If you:

  • find it difficult to attend an appointment during the working day;
  • are a busy parent with small children;
  • or rely on working carers to take you to appointments;

The Fylde Coast NHS has arranged for additional appointments to be provided outside of normal GP practice hours by the Fylde Coast Integrated Urgent Care Service.
